CBO Capacity Infrastructure Funding Tool

NYS 1115 Waiver creates a new process for billing and FFS payments for Health-Related Social Needs (HRSN) services.

  • The waiver also provides $500M in HRSN infrastructure investments intended to create Social Care Networks
    (SCNs) and help Community-Based Organizations (CBOs) build out or reconfigure their programs to expand capacity.
  • A similar 1115 Waiver in California created a Technical Assistance (TA) Marketplace where CBOs could easily apply for funding and support services to expand capacity in one centralized location.
  • Unlike California, the infrastructure dollars in NY will flow through SCNs and SCNs will be responsible for determining how to distribute the funding to best support capacity building.

Creating a CBO Marketplace

CBO Education
The tool helps educate CBOs by guiding them through a simple step by step process to model their potential revenue and expenses by HRSN service line with the new FFS funding instead of traditional grant-based funding.
Opportunity Identification
The tool analyzes current services compared to market benchmarks
to highlight potential opportunities for efficiency and growth.CBOs receive detailed guidance on potential uses of infrastructure funding and TA services matched to these opportunities.
Submitting Funding Requests
CBOs use the financial modeling and opportunity recommendations to articulate their requests for capacity-building funds in terms of how they will use the funding, projected growth in HRSN service volume and ROI of the funding dollars
Decision Support
SCNs utilize the tool for standardized assessment of individual requests from CBOs and can directly approve or deny requests or send back comments for rework to the CBOs in the tool.ROI scorecards and analytic dashboards in the tool help understand current and projected service volume across the network, TA services needed and relative value of funding requests.
