Clinical Integration

To achieve the three dimensions of the Triple Aim—enhanced patient experience of care, improved population health, and reduced per capita cost of health care—clinical integration among key stakeholders is crucial. That’s why our approach is to align payers and providers around a common vision, while providing the strategic processes to support a successful integration of ideas and actions.

We are experts in:

  • Evaluating and projecting market conditions and conducting competitive analyses, focusing on regulatory and business
drivers at the federal, state and local levels
  • Establishing and integrating complementary clinical processes and financial incentives across the continuum of care
  • Engaging physicians and other providers in transitioning from fee-for-service care to performance-based, accountable care models

“We needed help in developing our health care reform response strategies. COPE Health Solutions helped us create our long-term strategic initiatives. We are now developing an integrated health system and building the foundation for our strategic vision to manage population health across the continuum of care.”
Roland Fargo, president of medical network services, Adventist Health Southern California
