Dwayne Riley

Community Health Worker

Dwayne Riley is a Community Health Worker on the Enhanced Care Management (ECM) team at COPE Health Solutions. With extensive experience in healthcare administration and patient care, he specializes in transitional care coordination, addressing access barriers, and supporting high-need patients through referrals, education, and community engagement.

Previously, Mr. Riley served as a Certified Nursing Assistant and Administrative Assistant at organizations including Cedars-Sinai Medical Center, Providence Tarzana Medical Center, LA Care Health Plan, and Cantara Guest Home, where he honed skills in documentation, record-keeping, workflow optimization, and team training.

He holds a BS in Kinesiology from California Baptist University, is pursuing a Doctor of Occupational Therapy (OTD-S) at USC, and maintains certifications as a CNA, Certified Medical Administrator (Title 22/CDPH), BLS, CPR, and First Aid.

Mr. Riley brings empathy and efficiency to his work, inspired by live music, travel, softball, and bowling.
