Viviana Rivera
Patient Care Navigator
Viviana Rivera is a dedicated Patient Care Navigator with Cope Health Solutions who is passionate about helping individuals feel supported and empowered in their healthcare journey. In her role, she serves as the main point of contact for clients enrolled in the CalAIM Enhanced Care Management and Community Supports Program, providing both telephonic and field-based case management services.
Ms. Rivera takes pride in building meaningful, trusting relationships with her clients. She works closely with each individual to help them stay engaged in their medical care, adhere to medications, and overcome barriers that may prevent them from accessing the services they need. She collaborates with physicians, nurses, and other members of the care team to ensure clients receive coordinated, compassionate care.
Ms. Rivera began her career in healthcare as a Medical Assistant after graduating in 2023. She was offered a position at the facility where she completed her clinical training, assisting physicians with medications, injections, and direct patient care. She later worked in an ophthalmology practice that also included primary care services, where she had the opportunity to participate in community health outreach. One experience that deeply impacted her was helping patients with elevated A1C levels find appropriate meal plans to improve their health and prepare for surgery. Through this work, she discovered her passion for serving underserved communities and advocating for those navigating complex healthcare systems.
