Patient Care Navigator
Noé Ortega is a Patient Care Navigator at COPE Health Solutions. In this role, he will be the first point of con-tact helping clients navigate the health care system and remove possible barriers to care. Mr. Ortega works to serve community members by connecting them with the appropriate resources needed for health education, transportation, childcare and more. Mr. Ortega is part of a team consisting of community health workers, case managers, and healthcare administrators to best address the unique needs of individual community members.
Prior to becoming a Patient Care Navigator, Mr. Ortega completed both the Health Scholars and Care Navi-gator programs. As a Care Navigator, he worked on a team that focused on hypertension management. Du-ties included outreach to schedule and remind patients of future appointments. As a Health Scholar, he completed over 200 hours assisting health care workers in clinical settings where he gained valuable insight of how the health care system operates. The hands-on training helped prepare Mr. Ortega for his next role as a Patient Care Assistant at Loma Linda University Health Systems.
As a Patient Care Assistant, he provided support for the health care team by assisting patients with activities of daily living to promote a healthy lifestyle. While working in a hospital, Mr. Ortega was able to improve his time management skills, refine oral communication skills and strengthen his attention to detail. Being bi-lingual allowed Mr. Ortega to advocate on behalf of patients thus improving quality of care and establishing a clear line of communication. He plans to continue working within his community to facilitate efficient access to resources and enhance public health.
Mr. Ortega earned his Bachelor of Arts degree in psychology from the University of California, Riverside.
