Alizandra Mompar

Patient Care Navigator

Alizandra Mompar is a Patient Care Navigator at COPE Health Solutions. In this role, Ms. Mompar works with underserved communities to improve health outcomes by connecting patients to educational resources, community-based opportunities, and more. She collaborates closely with care coordinators, social workers, healthcare administrators, and case managers to support interventions targeted towards special populations, including homeless individuals and those diagnosed with chronic conditions.

Prior to her current role, Ms. Mompar gained valuable experience advocating for health equity through various volunteer positions with hospitals and nonprofit organizations. Her volunteer work involved collaborating with multidisciplinary teams to support underserved populations, addressing barriers to healthcare access, and promoting community health initiatives. This hands-on experience deepened her understanding of the systemic challenges faced by marginalized communities and reinforced her commitment to health equity.

In 2023, Ms. Mompar took on the role of Health Science teacher for 1st and 2nd graders, where she designed and delivered engaging lessons tailored for young children. She introduced them to key health concepts like nutrition and exercise, with the goal of fostering lifelong healthy habits and a solid understanding of wellness from an early age. By creating a supportive and interactive classroom environment, Ms. Mompar made complex health topics easy for her young students to grasp and relevant to their everyday lives, laying a strong foundation for their future health education.

Ms. Mompar earned a Bachelor of Science in Psychological & Brain Sciences from the University of California, Santa Barbara, and participated in the Technology Management Program. During her studies, she was involved in Psychobiological research, which allowed her to explore the intersections of psychological and biological processes and their impact on human health and behavior.
